A surgeon removes fat with liposuction from selected donor areas, processes the harvested fat, and transfers selected fat to the buttocks. The donor areas, amount transferred, and contour plan depend on the patient's anatomy and goals.

A BBL is not the same as a buttock implant or a nonsurgical injection. Ask which approach is being proposed, what it can and cannot change, and what result is realistic for your starting anatomy.

Florida law sets specific requirements for gluteal fat grafting performed in an office surgery setting. The statute says fat may be injected only into the subcutaneous space above the fascia overlying the gluteal muscle; intramuscular and submuscular injections are prohibited.

The same statute requires ultrasound guidance, or another technology authorized by rule that meets or exceeds it, while the cannula is placed and moved for fat injection. It also requires the physician to perform the fat extraction and injection and limits the operating physician to one patient throughout the procedure phases specified in the law.

These requirements are a safety baseline, not a guarantee against complications. Rules can change, and the care setting matters, so confirm the current law and exactly how it applies to your planned procedure.

Risks can include anesthesia complications, bleeding, infection, fluid accumulation, poor wound healing, pain, numbness or other sensation changes, asymmetry, fat necrosis, blood clots, heart or lung complications, unfavorable scarring, and the possibility of revision surgery. Gluteal fat grafting also carries the risk of fat embolism, which can be fatal. This is not a complete list.

No photograph, review, or consultation can guarantee a particular result. A qualified surgeon should explain the benefits, alternatives, limitations, and patient-specific risks before you sign consent forms or schedule surgery.

Recovery instructions vary with the operation and the patient. Before surgery, get a written plan for positioning, activity, incision care, garments, medications, warning signs, follow-up, transportation, and help at home or at your hotel.
